I'll supply the FD258 card if needed, or use the ones you have been instructed to fill out and submit.  Basic fee is $35 plus travel fee, determined by distance from St. Paul.  I will meet you at a location convenient to you and provide a receipt to submit for reimbursement.  It's a simple procedure, I will need to identify you first so bring your current government-issued photo ID.
